Registered Nurse - Case Manager
Case Managers work collaboratively with Physicians, Medical Directors, Patient Care Coordinator, Social Worker, Chaplain and Certified Nursing Assistants as an integral member of the interdisciplinary team. Case Managers assume the overall responsibility for organizing patient care, develop the interdisciplinary plan of care, provide comprehensive nursing assessments, teach appropriate palliative interventions to family members, assess and implement a pain/symptom management program to enhance quality of life and provide the highest level of hospice care to our patients.
Requirements: Licensed Registered Nurse, in good standing in the State of California required
Hospice experience preferred
Must possess excellent interpersonal skills particularly with family cultural/sensitivity
Current CPR certification
Valid driver’s license and current automobile insurance
Reliable transportation

Registered Nurse - Visiting
Performs comprehensive nursing assessments, teach appropriate palliative interventions to family members, assess and implement a pain/symptom management program to enhance quality of life and provide the highest level of hospice care to our patients.

Spiritual Counselor/ Chaplain
The Chaplain is responsible for assessing the spiritual and religious needs of patients and their families, arrange for the provision of spiritual and religious support in accordance with their wishes, and supporting families in the bereavement process.Must

Licensed Vocational Nurse - Visiting
Provide nursing services, including nursing assessments, treatments and diagnostic and preventive procedures as assigned. Observes for changes in condition, signs and symptoms of symptoms out of control and reports to the physician and RN reactions to treatments, including drugs and changes in the patient's physical or emotional condition. Provides teaching and education to the patient and family regarding the nursing care needs. This position reports to the RN Team Manager and DPCS. This is a part time position.

Licensed Vocational Nurse - Team
Works alongside the RN Case Manager as a team to provide nursing services, including assessments and treatments. Observes for changes in condition, signs and symptoms of symptoms out of control and reports to the physician and RN reactions to treatments, including drugs and changes in the patient's physical or emotional condition. Provides teaching and education to the patient and family regarding the nursing care needs. This position reports to the RN Case Manager and DPCS.

Medical Social Worker
Assesses the psychosocial status of patients and families/caregivers related to the patient’s terminal illness and environment and communicates findings to the registered nurse and other members of the interdisciplinary group. Provides an assessment in the patient’s identified residence and assistance when this is not safe and another plan is required. Carries out social evaluations and plans intervention based on evaluation findings. Counsels patient and family/caregivers as needed in relationship to stress, and other identified coping difficulties. Maintains clinical records on all patients referred to social work. Provides information and referral services for organization patients and families/caregivers regarding practical and environmental needs. Provides information to patients and families/caregivers and community agencies. Serves as liaison between patients and families/caregivers and community agencies. Maintains collaborative relationships with organization personnel to support patient care. Maintains and develops contracts with public and private agencies as resources for patient and personnel. Participates in the development of the individualized plan of care and attends regularly scheduled interdisciplinary group meetings. Actively participates in quality assessment performance improvement teams and activities. Reports to the DPCS.
Must have a master’s degree in social work by accredited by the Council on Social Work Education.
MSW Preferred.
